Annual membership
$350per person
for 12 months
Additional family members may enroll for $300 per person per year when the family enrolls together.
Your membership includes
- 2 checkup exams per year
- 1 emergency exam per year
- 2 regular cleanings or 3 periodontal maintenance cleanings after scaling and root planing per year
- All necessary dental X-rays
- Preventive care planning
